The Disadvantages of Wire Guided Localization

Wire-guided localization (WGL) has been the standard for surgical localization since its introduction in the 1970’s. It involves the placement of a thin, flexible wire (similar in size to a fuse wire or fishing line) into the breast prior to surgery. This wire serves as a precise guide for the surgeon, ensuring accurate identification of the targeted area during the procedure.

What are the disadvantages of wire guided localization? 

  • Timing constraints: The procedure must be performed either on the day of surgery or the day prior, which limits scheduling flexibility.
  • Risk of wire dislocation: Proper execution requires advanced navigation skills to minimize the risk of displacement.
  • Patient discomfort: The procedure may cause discomfort for the patient.

How is the Pintuition Marker® Placed? 

The Pintuition Marker® is pre-loaded into a sharp 14-gauge needle with an ultrasound-enhanced tip. It is secured by the needle against a locking mechanism, providing tactile resistance upon deployment. To place the marker, first visualize the target tissue using the preferred imaging modality, such as ultrasound. Apply local anesthesia to the area, then remove the protective sheath from the needle immediately before use. Insert the needle and slowly advance it until the tip is clearly visualized within the target tissue. Once the needle tip is confirmed in position, remove the spacer and deploy the marker by gently pushing the green stylet handle until slight resistance is felt. Continue advancing until the marker is fully deployed. Finally, retract the needle using a rotating motion and verify the marker's placement with an ultrasound or mammogram.
